Hollow cheeks can make the face appear tired, drawn or less balanced, even when you feel well-rested and confident. The best treatments for hollow cheeks are not about creating exaggerated volume. They are about restoring gentle support in the right areas, so your features look refreshed, defined and still recognisably you.

Cheek hollowing is a common concern at different stages of life. Some people naturally have a more sculpted facial structure, while others notice volume loss through ageing, weight changes, stress, illness or changes in skin quality. The right approach depends on why the hollowing has developed, the shape of your face and the result you would feel comfortable with.

Why do cheeks become hollow?

The cheeks are supported by fat pads, bone structure, skin, muscle and collagen. Over time, facial fat can shift or reduce, the skin’s natural collagen levels decline and the mid-face may lose some of its youthful lift. This can create a shadow beneath the cheekbone, a flatter mid-face or a more pronounced appearance around the nose-to-mouth lines.

For some clients, hollow cheeks are simply genetic. A naturally lean face can look beautifully defined, but it may also make under-eye shadows or signs of fatigue more noticeable. A recent loss of weight can have a similar effect.

This is why a good assessment looks beyond one dip or line. Treating only the hollow itself is not always the answer. In many cases, carefully supporting the upper cheek or mid-face produces a softer, more balanced result than filling deeply into the hollowed area.

Best treatments for hollow cheeks

Dermal filler for immediate cheek support

Dermal filler is often the most direct non-surgical option for hollow cheeks. A hyaluronic acid-based filler can be placed strategically along the cheekbone, in the lateral cheek or through the mid-face to restore lost structure and reduce the appearance of shadowing.

The appeal is clear: results are visible straight away, although there can be mild swelling initially. It may also create a subtle lifting effect around the lower face by improving mid-face support.

The amount of filler needed varies considerably. A client with early volume loss may need a very conservative treatment, while someone with more established hollowing may benefit from treatment built gradually over more than one appointment. More product is not automatically better. The aim should be a smooth transition between the cheek, under-eye and lower face, with results that suit your own features.

Hyaluronic acid filler is temporary. Longevity depends on the product used, the treatment area, your metabolism and lifestyle, but many clients enjoy results for around 9 to 18 months. It can also be dissolved if clinically necessary, which offers an added level of reassurance for suitable candidates.

Collagen-stimulating treatments for gradual improvement

Where hollow cheeks sit alongside thinner, less firm or crepey-looking skin, collagen-stimulating treatments can be a useful part of a longer-term plan. These treatments are not designed to replace significant volume in the way filler can, but they can improve skin quality and help the face look fresher over time.

Radio frequency microneedling uses controlled needling alongside radio frequency energy to encourage collagen and elastin production within the skin. It can be particularly valuable where the cheeks look less supported because the skin has lost firmness. Results develop gradually, with a course of treatment often recommended for the best response.

CO2 laser rejuvenation may also be considered for clients whose concern is linked to texture, fine lines, sun damage or more noticeable skin laxity. By resurfacing the skin and stimulating collagen renewal, it can improve the quality of the skin over the cheeks. It will not replace missing facial volume, so it is often best viewed as a complementary treatment rather than a standalone solution for deeper hollows.

For the right person, combining volume restoration with skin rejuvenation can be more effective than relying on a single treatment. The sequence matters, however. Your practitioner should advise whether to address structure first, improve skin quality first or create a plan that safely incorporates both.

Surgical fat transfer for longer-lasting volume

Fat transfer is a surgical procedure in which fat is taken from one part of the body and placed into the face. It can offer longer-lasting volume for suitable candidates, but it involves more downtime, a more involved recovery and the realities of a surgical procedure. Some of the transferred fat may be reabsorbed, so outcomes can be less predictable than people expect.

This option may suit someone seeking a surgical route, but it is not necessary for everyone with hollow cheeks. Non-surgical treatments can provide a refined and meaningful improvement without the recovery associated with surgery.

What will not reliably fix hollow cheeks?

Skincare is valuable for maintaining hydration, supporting the skin barrier and protecting against further collagen loss, particularly when used alongside daily SPF. However, even excellent skincare cannot restore a meaningful loss of cheek volume. Similarly, facial exercises may improve awareness of facial muscle movement but do not replace lost fat pads or rebuild structural support.

A healthy, stable weight can make a difference where recent weight loss has contributed to facial hollowing. That said, trying to gain weight specifically to fill the face is not a predictable or appropriate aesthetic strategy. Facial volume distribution is individual, and overall health should always come first.

Choosing the right treatment plan

The safest and most flattering results begin with a detailed consultation. Your practitioner should assess your facial proportions from different angles, your skin quality, existing volume, medical history and the changes you would genuinely like to see. Photographs may be used to support planning and track subtle progress over time.

It is also useful to discuss whether you want an immediate improvement, gradual skin rejuvenation or a combination of both. A client preparing for an occasion may prefer a conservative filler treatment with time for any swelling to settle. Someone focused on long-term skin quality may choose to begin with radio frequency microneedling or laser rejuvenation and review volume later.

Be open about previous filler, particularly if it was placed in the cheeks, tear trough or mid-face. Existing product can affect the safest treatment approach. A qualified practitioner should never assume that adding more filler is the right answer without first considering what is already present.

Safety, side effects and realistic expectations

Dermal filler is a medical aesthetic treatment and should always be carried out by a fully trained, insured practitioner who understands facial anatomy and has an appropriate plan for managing complications. Common short-term effects include tenderness, redness, swelling and bruising. These usually settle, but you should allow time before major events.

More serious complications are uncommon but possible, including vascular occlusion. Urgent assessment is required if there is severe or increasing pain, blanching or unusual skin colour changes after treatment. Choosing an experienced practitioner, attending an in-person consultation and following aftercare advice are essential parts of reducing risk.

Not everyone is suitable for treatment at every appointment. Active skin infections, certain medical conditions, pregnancy and breastfeeding, or recent dental work may mean treatment should be postponed or adapted. Honest consultation is never a barrier to beautiful results. It is how treatment stays safe and tailored to you.
